Photographer Information Form

Prepping me for your big day is one of the best ways to ensure a memorable wedding day album. Most wedding photographers use a "Shot List", which are very generalized, and don't take into consideration the specific needs of the couple. These shot lists only cover the basics, such as; "Getting Ready", "Ceremony", and "Reception". This form, however, is meant to give you a chance to really think of what details will make this wedding uniquely yours, so that I can give you something unforgettable to look back on.

Please take the time to fill in as much information as you can.
